Welcome!
Should students feel ill at any time during school, they must notify his/her teacher of the condition. The teacher will provide a pass to go to the office. The office will determine what should be done. No medicine will be dispensed without an "Administration of Medication at School" form filled out and signed by both the doctor and parent. Emergency first aid will be given when needed. If you are injured, notify the teacher immediately, so that necessary help can be given. In the event that an ambulance must be called for a student, the parents must assume the financial responsibility for the cost.
Illinois School Health Exam Requirements for 2024-2025
Kindergarten
1. Physical Examination and Immunizations- on proper Illinois form completed within a year prior to the start of school. (Parent must complete the health history section questions, sign, and date)
DTP/DTap- Four or more doses with the last dose qualifying as a booster and received on or after the 4th birthday
Polio- Four or more doses with the last dose qualifying as a booster and received on or after the 4th birthday
Measles, Mumps, Rubella (MMR) - Two doses of MMR or two doses each of separate Measles, Mumps and Rubella vaccines
Varicella (Chicken Pox) - Two doses of varicella vaccine or proof of having chicken pox
Hepatitis B- Valid 3 dose series
Lead Screening Test- 60436 is a high risk zip code and students must have a BLOOD test to check for lead levels.
Diabetes Screening-All students must be screened for diabetes through a few questions asked by a medical provider.
2. Vision Exam- on a proper Illinois form completed by an eye doctor within a year prior to the start of school (Parents signature is required on this form)
3. Dental Exam- on a proper Illinois form completed by a dentist within 18 months prior to May 15th of the school year
2nd Grade
1. Dental Exam- on a proper Illinois form completed by a dentist within 18 months prior to May 15th of the school year
6th Grade
1. Physical Examination and Immunizations- on a proper Illinois form completed within a year prior to the start of school (Parent must complete the health history section questions, sign, and date)
Tdap booster- for pertussis/whooping cough
Meningitis Vaccine- one dose received on or after the child’s 11th birthday
Diabetes Screening- All students must be screened for diabetes through a few questions
asked by a medical provider.
2. Dental Exam- on a proper Illinois form completed by a dentist within 18 months prior to May 15th of the school year
First time Illinois Transfer Students of any grade
1. Physical Examination and Immunizations- on a proper Illinois form completed within a year prior to the start of school. (Parent must complete the health history section questions, sign, and date)
2. Vision Exam- on a proper Illinois form completed by an eye doctor within a year prior to the start of school (Parents signature is required on this form)
3. Any specific health requirement for K, 2nd, or 6th graders
We ask that all required examinations are turned in prior to the first day of school. In compliance with Illinois state law, if immunizations and physical are not up to date and on file at school by Oct 15 of the school year your student will be excluded from school until immunizations and physical are up to date and on file.
Medication Requirements
Taking medication, prescription or over the counter, during school hours or during school-related activities is prohibited unless it is medically necessary for a student’s health and well-being. There may be some situations or medical diagnosis that require a student to take medication during school hours. If your physician determines that it is medically necessary for your student to take medication during the school day, Rockdale school does have specific paperwork that must be completed.
In order for the nurse or school staff to store and/or administer medications in the health office an Administration of Medication at School Form with physician’s signature must be completed and on file in the health office. Please note there is both a physician’s section and a parent’s section that must be completed and signed on this form. Medications must be in the original container marked properly with the student’s full name. A new form is required for each new school year and anytime a change in medication occurs. Forms can be picked up from the school health office or found under health forms on the school’s website
Students may be allowed to carry their own inhaler or Epipen on themselves with parents signed permission. If you and your physician feel your child is responsible and independent in self-administration and management, please complete and return the Student Self-Carry Form for Medication. A new form is required for each new school year. Forms can be picked up from the school health office or found under health forms on the school’s website
All students with asthma should provide an Asthma Action Plan to the health office before the beginning of each school year or as changes or new diagnosis are made. An Asthma Action Plan is a written plan that is developed with your child's physician to help control your child's asthma. An Asthma Action Plan form can be picked up from the school or found under health forms on the school’s website. Many physicians have their own forms that are also acceptable.
